If your insurance does not pay for the treatment rendered, the balance will be your responsibility. It is important that patients provide us with accurate insurance information in order for us to bill them. If your insurance company requests proof of treatment rendered we will gladly provide x-rays and/or a written report explaining the reason for treatment.
